Black Out	License Requirements field ("Eyeglasses, etc." need not be blacked out.) *If information other than the above is blacked out, the Bank cannot verify the validity of the identification and proceed with the procedures. *[Individual customers] If using a smartphone, please send the image as it was taken without blackening out any fields.
Notes	Please also make a copy of the back side if any changes are indicated. Make sure that the expiration date, the seal of the Public Safety Commission, etc. appear clearly in the copy. An address for which mail forwarding has been requested cannot be registered as the current address.

• Please make sure that the copy or image shows the entire document clearly. (Any copies and images that are unclear or have edges cut off are unacceptable.)

• False identity information may be subject to punishment under the Act on Prevention of Transfer of Criminal Proceeds.
